If you change the rule place=city ... to place=city & name=* ...
I am using the default style in mkgmap, so isn't that a bug in this default style's rule? Shouldn't it be corrected in mkgmap?
But this sounds like a bug in the OSM data. Could you give me some examples, where you have duplicate POIs? Then I can check if this is an OSM data bug or how to better solve this.
