the best way to get a good index is to use the bounds parameter.
The input file can be found here:
http://www.mkgmap.org.uk/download/mkgmap.html

http://osm.thkukuk.de/data/

example usage:
--bounds=bounds-latest.zip

See also the --location-autofill option.

To verify the result you can use echotags in the style, e.g.
highway=living_street { echotags "This is a living_street" }
