Mark, I can confirm the problem in mapsource with today's data and mkgmap/splitter versions (see also my latest post for other details). I can send you a snapshot with the mapsource error. I now have a map of Italy in mapsource. Rome is all contained within a single tile. I have tried lots of routes within that tile without any problems. I shall also try the map on a Nuvi.
For your information, here's the mkgmap options I used:

I'll let you know how the Nuvi copes with the map...
